Nominate a former Cal Poly Athlete, Supporter or administrator.  Nominations for the 2011 Hall of Fame are now being accepted.  For copy of the 2011 Nomination Form, please click here. Nominations are considered in March of each year and selections made by April of that year.  For nominations to be considered they must be submitted in full no later than Febraury 28th of each year.  Incomplete Nomination Forms will not be considered, neither will letters of nominations that are sent without the Nomination Form.
